What is a Medicare Chronic Disease Management Plan?

A Chronic Disease Management (CDM) plan is a Medicare program designed to support people living with chronic or complex medical conditions.

A chronic condition is generally defined as a condition that:

• Has lasted 6 months or longer, or
• Is expected to last longer than 6 months

Through this program, your GP can refer you to allied health professionals such as Exercise Physiologists to help manage your condition.

Medicare allows up to 5 allied health sessions per calendar year under this program.

These sessions can be used with Exercise Physiology or shared between other allied health providers such as physiotherapy, podiatry or dietetics.

How to Access Medicare Exercise Physiology

Getting started is simple.

Step 1 – Visit your GP

Speak with your GP about whether you are eligible for a Chronic Disease Management (CDM) plan.

Step 2 – GP prepares your referral

Your GP will prepare your CDM plan and referral to Exercise Physiology at MK Movement.

Step 3 – Book your appointment
